First Time Applicants

Additional Requirements


For Married Women Who Wish To Change to Married Name:
  • NSO Marriage Contract - original and photocopy
  • Report of Marriage duly authenticated by NSO if married abroad

Applicants who availed of Dual Citizenship
  • Identification Certificate
  • Oath of Allegiance
  • Order of Approval
  • Report of Birth (for those born abroad)


For Minor Applicants (Below 18 years old)
  • Confirmed appointment (except for 1 year old and below)
  • Personal appearance of the minor applicant
  • Personal appearance of either parent (if minor is a legitimate child) / of mother (if minor is an illegitimate child)
  • Original NSO Birth Certificate
  • Valid photo ID of minor (School ID or Form 137 with dry seal)
  • NSO marriage certificate of parents
  • Original and photocopy of valid passport of the person traveling with the minor
  • Photocopy of valid passport of either parent (if minor is a legitimate child) / of mother (if minor is an illegitimate child)

If minor is not traveling with either parent or alone
  • Affidavit of support and consent
  • DSWD Clearance

Other Important Pointers:
  • Please do not deal with fixers
  • Apply for your passport at least 12 weeks before your planned trip
  • Make sure your identifications are valid
  • Bring extra photocopies of your documents
  • Passport can be delivered to your home or office for a fee of Php 120.00

Renewal of Passport

  1. Personal appearance
  2. Confirmed appointment (if applying in Manila)
  3. Completed application form
  4. Valid IDs with photo and supporting documents
  5. Most recent expiring or expired passport





Applying Passport in USA


There are two (2) ways to apply for passport in USA:

  1. Go to the nearest Philippine Embassy/Consulate
  2. Wait for a Consular Outreach near your home state


Requirements:

  1. Duly-accomplished Application Form
  2. Latest passport
  3. Photocopy of the biographic page of passport
  4. Photocopy of Birth Certificate
  5. Proof that applicant has not applied for foreign citizenship, e.g. resident alien card (green card)
  6. Prepaid, self-addressed return envelope, if Passport is to be mailed back.
  7. Passport fee of $60.00, payable in cash or money order
